Impact of Inpatient Initiation of SGLT2 Inhibitors on Diuretic Requirements in Patients With Heart Failure.

Abstract
This study focuses on the impact of initiating sodium-glucose cotransporter 2 inhibitors (SGLT2is) on loop diuretic dose adjustments in hospitalized patients with known or new heart failure (HF). In a retrospective analysis of 61 patients started on empagliflozin or dapagliflozin during hospitalization or at discharge, 40% required changes in loop diuretic dosing within 60 days post-discharge, with 29% needing dose reductions. No significant adverse effects on renal function were observed, and discontinuation of SGLT2is was infrequent. The findings suggest potential for diuretic dose reduction following SGLT2i initiation but highlight the need for further research to establish clear clinical guidelines.
