Malpositioned Implant in the Esthetic Area: The Peri-Implant Plastic Surgery and Angulated Welded Abutment Approach.

Abstract
This article focuses on the treatment of malpositioned dental implants in the esthetic zone using a novel angulated welded abutment (AWA) approach combined with peri-implant plastic surgery. The AWA technique involves digitally designing two titanium components that are welded together with an angulated screw housed inside, allowing correction of implant angulation up to 45 degrees and optimal positioning of the screw hole relative to the gingival margin. This design facilitates soft tissue creeping over the abutment, enabling improved esthetic outcomes through mucogingival surgery without implant removal. While the approach shows promise in managing implants with excessive buccal angulation, the authors note that further long-term clinical studies and standardization are needed before routine clinical application.
