Frequency and clinical outcomes of pharmacist-driven switching from warfarin to direct oral anticoagulants in an underserved patient population: A retrospective cohort study.

Abstract
This article examines pharmacist-driven switches from warfarin to direct oral anticoagulants (DOACs) in an underserved patient population at a federally qualified health center. Among 189 adult patients assessed, 57% were switched to a DOAC primarily due to less frequent monitoring requirements and patient preference, while reasons against switching included clinical inappropriateness (e.g., mechanical heart valve, high body mass index) and patient choice. The study found low incidences of thrombotic and bleeding events within six months post-assessment and demonstrated adherence to recommended laboratory monitoring after switching. These findings suggest that pharmacist-led anticoagulation management using a standardized electronic health record template can safely facilitate warfarin-to-DOAC transitions in underserved settings.
