Abstract
This paper describes how community-oriented integrated practice (COIP) is moving forward the idea of comprehensive primary healthcare as described at the 1978 international conference at Alma Ata. COIP emphasises how to think as well as how to act to achieve collaboration for health and care at local level. The paper explains that rectifying common misunderstandings of Darwin's theory of evolution, understanding the importance of context, and the nature of health itself could help us to see why our everyday habits of linear thinking fall short of being able to achieve community-oriented integrated practice.
