Early outcomes after post-cardiotomy extracorporeal membrane oxygenation in paediatric patients: a contemporary, binational cohort study.

Abstract

This study evaluates early outcomes and risk factors for pediatric patients requiring post-cardiotomy extracorporeal membrane oxygenation (PC-ECMO) after cardiac surgery in Australia and New Zealand, using data from the Australia and New Zealand Congenital Outcomes Registry for Surgery (ANZCORS). Among 12,290 patients under 18 years who underwent cardiac surgery between 2013 and 2021, 3% required PC-ECMO, with a hospital mortality rate of 35.6% and two-thirds experiencing major complications. The Norwood procedure and modified Blalock–Taussig shunts had the highest incidence of PC-ECMO use, with single-ventricle physiology and intracranial hemorrhage identified as significant predictors of hospital mortality. The study highlights that despite advances, PC-ECMO remains associated with substantial morbidity and mortality in this population.
