Prophylactic epicardial pacemaker implantation in tricuspid valve replacement.

Abstract
This article focuses on evaluating the benefits and risks of prophylactic epicardial pacemaker implantation in patients undergoing surgical tricuspid valve replacement (TVR). In a retrospective study of 80 patients without prior pacing devices, 46% required cardiac pacing postoperatively due to atrioventricular conduction disorders, which were difficult to predict preoperatively. The safety profile was acceptable, with a 2.5% rate of life-threatening complications related to the epicardial pacemaker. Given the high incidence of conduction disturbances and challenges associated with transvenous pacing through a tricuspid bioprosthesis, the study supports the use of prophylactic epicardial pacing during TVR, while noting the need for further research on alternative pacing strategies.
