Typhoid fever in a migrant farmworker in New York State, April 2022.

Typhoid fever is a systemic infectious disease caused by the bacterium Salmonella Typhi and is rare in the United States. In April 2022, a case of typhoid was confirmed by positive blood cultures in Orleans County, New York among a 40-year-old Hispanic male who recently traveled from Mexico. He was employed as a seasonal farmworker and resided in migrant housing in Orleans County. Sixteen household contacts were identified and tested for possible typhoid infection. Because the migrant workers had not yet identified primary care providers in the area, the Orleans County Health Department coordinated testing services for the case and contacts. Salmonella Typhi was not detected in any of the case or contacts' stool samples, indicating the case was not a carrier and no transmission occurred to contacts. This report is intended to encourage local health agencies to consistently prioritize the health of the migrant worker population.
