Evaluation of a Statewide Online, At-Home Sexually Transmitted Infection and Human Immunodeficiency Virus Screening Program.

Abstract
This article evaluates the first year of Test Yourself Colorado (TYC), an online, at-home program providing free HIV self-testing and triple-site gonorrhea (GC) and chlamydia (CT) testing (urogenital, rectal, and pharyngeal) to adults in Colorado. Among 1790 clients, the program reached populations at risk, including men who have sex with men (MSM), with 41.3% of MSM clients reporting no HIV test in the prior year; 9.6% of returned STI tests were positive, with higher positivity in MSM and younger clients. The study identified lower STI test kit return rates among Hispanic clients and found that extragenital testing, especially rectal testing, had higher positivity rates, while pharyngeal testing showed limited utility in women. The TYC program facilitated rapid notification and treatment, often via telehealth, but highlighted the need to address disparities in utilization and improve follow-up and communication strategies to ensure equitable access and outcomes.
