Thermosensitive multivesicular liposomal hydrogel: a potential platform for loco-regional drug delivery in the treatment of osteomyelitis caused by antibiotic-resistant biofilm-forming bacteria.

Abstract

This article focuses on the development of a localized, biodegradable, and biocompatible drug delivery system using vancomycin hydrochloride (VAN HL)-loaded multivesicular liposomes (MVLs) incorporated into a thermosensitive hydrogel composed of poloxamer 407 (P407) and hyaluronic acid (HA) for treating biofilm-mediated osteomyelitis. The optimized hydrogel system demonstrated sustained antibiotic release for up to three weeks, exhibited shear-thinning behavior at body temperature (37°C), and effectively inhibited and eradicated biofilms formed by Staphylococcus aureus and methicillin-resistant S. aureus (MRSA) in vitro. This injectable in situ gel system offers a potential alternative to current treatments that often require multiple surgeries, addressing limitations of non-biodegradable carriers like polymethyl methacrylate (PMMA). Further preclinical studies are suggested to evaluate its clinical applicability in osteomyelitis management.
