Abstract

This article focuses on the role of radiation therapists in delivering total body irradiation (TBI) using helical tomotherapy, particularly in pediatric patients undergoing hematopoietic stem cell transplantation for conditions such as acute myeloid leukemia. Helical tomotherapy offers advantages over conventional linear accelerator methods by providing highly conformal, uniform radiation doses with improved patient comfort and reproducibility, especially through supine positioning and dual-orientation scanning for taller patients. Radiation therapists are integral throughout the treatment process, including patient counseling, immobilization, computed tomography simulation, treatment planning, positioning verification, and delivery, while managing challenges such as patient movement, dose homogeneity, and setup uncertainties. The article highlights differences in protocols between pediatric and adult patients, emphasizing stricter organ-at-risk constraints and additional immobilization measures for children, and underscores the importance of multidisciplinary collaboration to optimize treatment accuracy and patient outcomes.
