Hepatitis B–CpG Vaccine Series for Healthcare Workers Who Are Hepatitis B Vaccine Nonresponders.

Abstract
This study evaluated the immune response to Heplisav-B (HepB-CpG), a 2-dose hepatitis B vaccine with a CpG 1018 adjuvant (Dynavax Technologies Corporation), in healthcare workers (HCWs) who previously failed to respond to at least five doses of aluminum-adjuvanted hepatitis B vaccines. Among 49 HCWs with anti-hepatitis B surface antigen antibody (anti-HBs) levels below 10 mIU/mL, 91% (43/47) achieved seroprotection (anti-HBs ≥10 mIU/mL) after two doses of HepB-CpG, and 84% (41/49) responded after one dose. The vaccine was generally well tolerated, and response rates remained high even among participants with known risk factors for nonresponse, such as older age, immunosuppression, and diabetes. These findings suggest that HepB-CpG may be an effective alternative for HCWs who do not respond to traditional aluminum-adjuvanted hepatitis B vaccines.
