Abstract

The Centers for Medicare and Medicaid Services (CMS) is offering State Planning Grants to Promote Continuity of Care for Medicaid and CHIP Beneficiaries Following Incarceration. The grants aim to develop operational capabilities to ensure that individuals who are inmates of public institutions and eligible for medical assistance under the state Medicaid program or child health assistance under CHIP receive continuous care. The deadline for applications is November 26, 2024, and a total of $106 million is available for up to 56 awards of up to $5 million each. State governments are eligible to apply, and the funds can be used to address operational barriers and improve systems for continuity of care in various correctional facilities.
