Changing the Paradigm: Practical Wisdom as True North in Medical Education.

Abstract
This article focuses on the integration of practical wisdom into medical education as a guiding principle for developing competent and compassionate physicians. Practical wisdom, defined as the capacity to apply knowledge, technical skills, and virtues such as compassion, humility, and trustworthiness appropriately in complex clinical situations, is proposed as the "true north" for medical training. Drawing on philosophical models, particularly Ardelt's three-dimensional model of wisdom (reflective, cognitive, and affective), and informed by emerging insights from neuroscience, psychology, sociology, and human performance sciences, the article outlines a roadmap for cultivating wisdom through observation, positive emotion, cognitive flexibility, self-regulation, mentorship, and systemic change. It emphasizes that fostering wisdom requires longitudinal mentorship, reflective practice on critical incidents, and creating supportive learning environments, while addressing systemic barriers that impede wise medical practice.
