Peri-Implant Enhancement of the Breast: Imaging Features, Significance, and Management Strategies.

Abstract

This article focuses on the evaluation of peri-implant enhancement observed on contrast-enhanced breast MRI and its association with implant-related malignancy, specifically breast implant–associated anaplastic large cell lymphoma (BIA-ALCL). In a retrospective review of 100 patients with peri-implant enhancement, only 2% were diagnosed with BIA-ALCL, both involving textured implants, while 98% had benign findings such as capsular fibrosis or inflammation. The study highlights that peri-implant enhancement alone, especially without associated effusion, mass, or adenopathy, is a nonspecific finding with a low likelihood of malignancy, and recommends that management consider implant surface texture and clinical presentation, discouraging diagnostic capsulectomy in patients with smooth implants. Further prospective studies are encouraged to validate nonoperative management strategies for isolated peri-implant enhancement.
