Impact of antiresorptive agents on mortality risk in postmenopausal women with osteoporosis: insights from a nationwide cohort study.

Abstract

This article focuses on evaluating the impact of antiresorptive agents (ARs), including bisphosphonates and selective estrogen receptor modulators, on mortality risk among postmenopausal women with osteoporosis in South Korea. Using a nationwide cohort of 117,871 women, the study found that AR use for at least one year was associated with a significant reduction in all-cause mortality (hazard ratio [HR] 0.43), particularly lowering cardiovascular disease (CVD) mortality (HR 0.48) and injury- or fracture-related mortality (HR 0.46). The mortality benefits increased with longer durations of AR therapy, and the protective effects were most pronounced in women without preexisting hypertension or CVD. These findings suggest that sustained AR treatment may reduce mortality risks in this population, although the study's observational design and focus on a Korean cohort limit generalizability and causal inference.
