Abstract

This article focuses on examining the relationship between cognitive intra-individual variability (IIV), neurofilament light chain protein (NfL), and Apolipoprotein E ε4 (APOEε4) status in an older, diverse sample. Using data from the Health and Aging Brain Study: Health Disparities (HABS-HD), the study assessed whether NfL moderates the association between APOEε4 status—a genetic risk factor for dementia—and cognitive variability measured across multiple neuropsychological tasks. Results indicated a significant but small moderating effect of APOEε4 on the relationship between NfL levels and cognitive IIV, with a stronger effect observed in APOEε4 carriers. The authors suggest further research in populations with greater symptom severity to better understand these biomarker interactions.
