An Algorithm to Avoid Missed Bowel Injuries in Blunt Abdominal Trauma Patients.

Abstract
This article focuses on the development and evaluation of the Avoiding Missed Bowel Injuries (AMBI) scoring system, designed to improve detection of bowel injuries following blunt abdominal trauma (BAT). Implemented at a Level I trauma center, AMBI integrates imaging, physical exam, and laboratory findings into a point-based algorithm to guide surgical decision-making. In a 9-month trial involving 141 BAT patients, AMBI demonstrated high sensitivity (92.9%) and specificity (95.3%) for bowel injury detection, outperforming existing scoring systems such as the Bowel Injury Prediction Score (BIPS) and the Faget scoring system. Use of AMBI was associated with a significant reduction in hospital length of stay for patients with bowel injuries, although missed and delayed diagnoses still occurred. The study suggests further validation across multiple centers and integration with artificial intelligence to enhance diagnostic accuracy and patient outcomes.
