Abstract

This article focuses on the health and well-being of family caregivers of Native Hawaiian and Pacific Islander (NHPI) adults living with Alzheimer's disease and related dementias (ADRD), exploring cultural values and identifying barriers and facilitators to caregiving. Using a multimethod study with surveys and interviews of 11 caregivers across the United States, the research highlights caregiving as a communal and family-centered responsibility (kuleana) deeply rooted in cultural values such as lōkahi (balance) and the role of kūpuna (elders) as cultural knowledge holders. Barriers to caregiver health include high living costs in Hawaiʻi, limited resources for Compacts of Free Association (COFA) Islanders, distrust of the medical system, and stigma around external caregiving support, while facilitators involve engagement in cultural practices and strong family and community connections. The findings suggest that culturally tailored, family-centered interventions that emphasize relational self-care and community support may be critical to promoting caregiver health in NHPI communities.
