A Policy Analysis of WISH Act Implementation and Nursing Integration Strategies.

Abstract
This article analyzes the Well-Being Insurance for Seniors to be at Home (WISH) Act, which proposes universal social insurance for long-term care focused on activities of daily living (ADL) assistance for individuals aged 65 and older. It identifies a critical gap in the legislation's design: while the WISH Act addresses basic personal care needs, it does not adequately account for the clinical complexity of beneficiaries, 85% of whom have multiple chronic conditions requiring ongoing health monitoring beyond ADL support. The analysis advocates for strategic nursing integration—targeted involvement of registered nurses in complex case identification, care transitions, chronic disease monitoring, and family caregiver support—to complement existing Medicare services, reduce hospitalizations, and maintain cost neutrality. Using Russell and Fawcett's Conceptual Model for Nursing and Health Policy, the study highlights that such nursing integration can bridge the divide between social and medical care needs, improve equity in clinical oversight, and enhance the WISH Act's effectiveness without expanding its core ADL focus.
