Impact of pharmacist-provided Medicare annual wellness visits and chronic care management on reimbursement and quality measures in a privately owned family medicine clinic.

Abstract
This article evaluates the impact of pharmacist-led Medicare Annual Wellness Visits (AWVs) and Chronic Care Management (CCM) services on reimbursement and quality measures in a privately owned family medicine clinic. Following the integration of a clinical pharmacist working 30 hours per week, the clinic experienced increased revenue from AWVs and CCM, with AWV reimbursement rising by approximately $25,800 in 2018 and $26,400 in 2019 compared to 2017, and CCM reimbursement increasing by $16,600 in 2018 and $5,700 in 2019. Additionally, completion rates of Healthcare Effectiveness Data and Information Set (HEDIS) quality measures and overall star ratings improved during this period. The study suggests that pharmacist involvement in these services can fill care gaps, enhance preventive care delivery, and contribute to value-based payment models in small, independent primary care practices.
