Association Between Draft Order and Stress Sonography of the Ulnar Collateral Ligament of the Elbow in Professional Baseball Pitchers: An 18-Year Study.

Abstract

This article investigates whether professional baseball pitchers selected in earlier rounds of the Major League Baseball (MLB) draft exhibit greater adaptive or pathological changes in the ulnar collateral ligament (UCL) of the elbow, as measured by dynamic stress ultrasound (SUS). Analyzing 383 drafted pitchers over 18 years, the study found no significant association between draft round and relative UCL thickness, ulnohumeral joint laxity, or presence and progression of elbow pathology. Although increased UCL thickness correlated with the presence of pathology, the extent and progression of these changes did not differ by draft position, suggesting that higher draft status does not predict greater elbow adaptations or injury risk detectable by SUS.
