Reduced live birth rates following ART in adenomyosis patients: a matched control study.

Abstract
This article investigates the impact of adenomyosis, a benign uterine disorder, on live birth rates (LBRs) in women undergoing assisted reproductive technology (ART), specifically in vitro fertilization (IVF) and intracytoplasmic sperm injection (ICSI). In a matched cohort study of 285 women with MRI-confirmed adenomyosis and 285 controls matched for age and embryo quality, the adenomyosis group showed significantly reduced cumulative LBRs (41.4% vs. 51.9%) and clinical pregnancy rates (53.3% vs. 63.9%) after ART, with adenomyosis identified as an independent risk factor for lower live birth chances. Early miscarriage rates did not differ significantly between groups. The study highlights the importance of thorough clinical and imaging evaluation for adenomyosis in infertility management and suggests that adenomyosis primarily impairs implantation rather than later pregnancy stages, though further prospective research is needed to clarify underlying mechanisms and optimize treatment strategies.
